2025 BWHA - Add/Change Player Registration Form

For adding/changing/de-registering a player's registration, please fill out the following form.

This form is solely for the use of club secretaries.

All changes to registrations that are submitted after 12pm Thursday will not take effect until the following weekend’s fixtures.

For more than one player registration change, please submit multiple forms.

Submissions to change player registration close on 11th July.

Adding a player registration may occur at any time throughout the season.
